Donor receptor weight and gender mismatch can lead to kidney transplant failure
Release date: 2017-04-01 A new study suggests that the success of kidney transplantation may depend in part on the weight and sex of the kidney donor, and these factors are often not considered when selecting recipients of dead donor kidneys. The forthcoming results of the American Journal of Clinical Medicine (CJASN) indicate that current protocols based on immunological methods that match donors and recipients may need to be changed. Previous studies have shown that if the kidney donor is smaller than the recipient, the risk of kidney transplant failure may be higher, probably due to the increased strain of the relatively small transplant kidney. However, few studies have investigated results associated with donor and recipient body weight mismatches. There is also a suggestion that a gender mismatch between kidney donors and recipients may lead to worse outcomes after transplantation, but the study produced conflicting results. To investigate these issues, a team led by Dr. Amanda Miller and Dr. Dalhousie University of Canada and Karthik Tennankore of the Nova Scotia Health Authority examined whether kidney transplants from smaller heterosexual donors affected recipient transplant outcomes. . The researchers analyzed data from the US Deceased Recipient Queue, which was included in the Scientific Register of Transplant Recipients from 2000 to 2014. At a median follow-up of 3.8 years, 21,611 of 115,124 kidney transplant patients failed to transplant. After considering other transplant variables, the researchers confirmed that if the kidney transplant recipient weighs 30 kilograms (66 pounds) more than the donor, the risk of graft failure is higher than the equivalent weight of donors and recipients. If the kidneys are from a smaller heterosexual donor, the relative risk of graft failure is further increased to 35% of men receiving male kidneys from female donors and 50% of females receiving male donor kidneys. This risk is similar to the risk observed when a recipient receives a kidney transplant from a donor with diabetes, a known risk factor for renal failure.
