The team of Professor He Jianxing succeeded in putting a "edge". The lungs are "re-repaired" by extracorporeal lung perfusion and transplanted into the pharaoh with terminal lung fibers.

The first hospital of Guangyi Hospital has carried out lung transplantation since 2003. During the waiting period, Pharaoh's condition continued to deteriorate, and he developed non-invasive mechanical ventilation. His blood oxygen saturation was only 92% to 95%. Generally, the blood oxygen level of a normal person is maintained at 98% to 100%. Below 90%, the normal demand cannot be met, and below 85%, there is a danger. While Pharaoh had a little activity, blood oxygen immediately fell to 75%~85%, sweating, face, palms and bloodless, heart rate reached 120 to 30 times per minute.

"If you can't wait for the donor lung, Pharaoh is extremely dangerous every minute." Xu Xin, the chief physician of the Organ Transplantation Team of Guangyi Hospital, and his colleagues are constantly monitoring the pharaoh for 24 hours, hoping to save. The lungs of his life appeared as early as possible.

The anxious waiting for the pharaoh, finally waited for a turn last month - there is an organ donor's lungs to match him. Because of the great love of organ donors and family members, Pharaoh’s lungs have it. However, the tricky thing is that the donor has a traumatic injury. When he was rescued, he placed a chest tube on the pneumothorax and contracted the lungs on both sides. This situation is a typical “edge lung”.

Xu Xin said that the international consensus is that unless it is an emergency saver, it will not use "marginal lungs". Although the quality of the "marginal lungs" that Pharaoh waited for was not so ideal, but faced with the difficulty of breathing, and the urgent need to "change the lungs" to save lives, Pharaoh and his family unanimously decided to accept this valuable donation.

The Organ Transplantation Team of the First Affiliated Hospital of Guangxi Medical University implemented the “Extracorporeal Inoculation Technique (EVLP)”. Xu Xin said that this technology simulates the functional status of the lungs in the human body, accurately assessing and re-treating the quality and function of the lungs in vitro, so that the quality of the donor lungs is improved. After more than three hours of hard work, the transplant team finally allowed this valuable “edge lung” to be “repaired”.

When the medical staff was preparing to send Pharaoh to the operating room, his condition deteriorated again. Even if he wore an oxygen mask to absorb oxygen, his blood oxygen level was only 85%, and the danger was extremely high. After 4 hours of surgery, this "edge lung" was finally implanted into the chest of Pharaoh, for him to win the opportunity to prolong life. After 10 days of smoothness in the intensive care unit, Pharaoh was successfully extubated and transferred to the general ward last week. The situation is stable now, can speak softly, and the body is getting better step by step.

According to reports, this is the first case of "edge lung" successfully transplanted after extracorporeal lung perfusion.

Lung transplant experts pointed out that at present, China's lung donor resources are still in short supply, and the "preservation period" of donor lungs is shorter than other donors such as liver and kidney. The perfect lung source is very rare. Rational use of extracorporeal lung perfusion techniques to fully utilize the “marginal lung” resources will bring new hope to many patients with end-stage lung disease who are still waiting for lung transplantation and whose disease progresses rapidly.

What is "edge lung"?

"Edge lungs" often have pulmonary edema, lung contusion, etc. In organ harvesting tissue, about 1/5 to 1/6 of the lungs belong to the "marginal lung."

Unlike other organs, lung transplantation has special strict requirements for donor lungs to ensure postoperative quality. If the lungs donated are evaluated according to a set of indicators, the lungs can be rated as perfect, ideal, usable, and close to use, with the lungs being close to the unusable lungs being the marginal lungs.
